Just like anybody else, someone with complex healthcare conditions has lifestyle aspirations. It is important for them to focus on the things they can do, rather than the things they have lost.
ICCM provides complex home care services for adults who have secured Continuing Healthcare (CHC) funding following a care needs assessment by the NHS using a DST (decision support tool). We recruit bespoke healthcare support worker teams to enable people to lead lives as normal as possible, at home and within their own communities.
Our teams undergo specialist training so they can support the client with their daily living activies. This can range from managing their healthcare needs (for example breathing, nutrition and skin integrity) or helping them carry out their lifestyle choices – having an active social life, doing the things they want to do. We work with our clients to build a competent team of people who mould themselves around individual care needs, to enable home and community living.
We work with clients and their families to understand risks and ensure that the service we offer balances the need for safe care without suppressing aspiration!
