Achieving outcomes for our clients

We believe our service outcomes are measured in two primary ways:

  1. Helping our clients achieve their lifestyle choices
  2. In providing cost-effective services to those who fund these CHC packages

Our Clients

When our clients refuse to become defined by their condition and choose to get on with their lives, we know we are helping them to achieve the outcome they desire

Our clients tell us that the services we provide are different – ICCM is differentiated by the uniqueness of the care packages it can create;  allowing many to whom ‘the worst has happened’ achieve more

Our Commissioners

ICCM services are priced following a comprehensive clinical assessment to provide a bespoke care package based on individual client needs.  We provide a weekly or annual budget for each client to our Commissioners.

We constantly assess and review our inputs to reflect the changing needs of our clients, and try to reduce dependency upon our teams when possible, thus reducing costs.  Clients who need a high level of nursing input at the beginning of an episode of care may, according to progress, transition to care provided by a team of Healthcare Support Workers, which is more cost effective for Commissioners.  ICCM always welcomes opportunities to discuss creating cost-effective solutions to local needs with Commissioners.  We also participate in Commissioner-led contracting exercises to seek best cost solutions.

ICCM’s ethos and culture is about providing the best possible care; promoting lifestyle choices and achieving positive outcomes for every client.
